Ventricular Tachycardia with Narrow QRS
نویسنده
چکیده
gram. Tachyeardias originating in the left bundle branch and documented by right, left, and His bundle recordings have been produced in animals given excessive digitalis. We have noted a similar tachycardia in a patient with ischemic heart disease receiving digitalis during hypokalemia. The QRS complexes were 0.10 sec in duration and by contour suggested an ectopic focus located in the posterior fascicle of the left bundle; His bundle recordings were consistent with this diagnosis. As the ectopic rhythm became synchronized with a slightly irregular sinus rhythm, bidirectional depolarization of the His bundle with fusion His potentials could be demonstrated.

Narrow QRS Tachycardia with Alternate Wide QRS Beats: What is the Mechanism?
A 44-year old lady underwent electrophysiology study for recurrent palpitations and documented narrow QRS regular tachycardia. The baseline ECG showed subtle preexcitation that was easily manifest on atrial pacing. The retrograde atrial activation sequence during ventricular pacing was eccentric suggesting retrograde conduction over the accessory pathway. A regular narrow QRS tachycardia with c...
متن کاملNarrow QRS Complexes in a Patient with Baseline Surgical RBBB: What is the Mechanism?
A 26-year-old woman born with Tetralogy of Fallot initially underwent a left Blalock-Taussig shunt followed by a complete repair with a transannular patch at 2 years of age. She complained of non-sustained short episodes of palpitations. Her Holter monitor showed episodes of six beat run of non-sustained ventricular tachycardia. She was scheduled to undergo surgical pulmonary valve replacement....
متن کاملResponse of narrow QRS tachycardia to premature atrial extra. What is the mechanism?
A 60-year-old woman was referred for radiofrequency catheter ablation of narrow QRS tachycardia that was terminated with intravenous adenosine. Twelve-lead ECG showed no baseline preexcitation. Echocardiogram was essentially normal. The electrophysiological study showed a normal AH interval of 114 ms and HV interval of 48 ms during sinus rhythm. Anterograde study demonstrated no dual AV nodal p...
متن کاملTermination of a narrow QRS-complex tachycardia: what is the mechanism?
A 27-year-old man with a 10-year history of recurrent episodes of paroxysmal supraventricular tachycardia underwent an electrophysiologic procedure. The baseline sinus cycle length was 860 msec, and the atrial-His (AH) and Hisventricular (HV) intervals were 80 and 45 msec, respectively. A narrow QRS-complex tachycardia was induced by atrial overdrive pacing. The cycle length of the tachycardia ...
متن کامل[The use of adenosine in the diagnosis and treatment of cardiac arrhythmias].
The authors investigated the effect of adenosine or ATP on narrow QRS tachycardia in 56 pts, 3 pts with wide QRS tachycardia 9 pts with suspected latent preexcitation and 10 pts with PVC suspected to be ventricular parasystole. After the bolus iv. administration of adenosine or ATP every SVT was stopped related to AV node (44 pts), but in the rest twelve related to atrial origin of SVT only one...
متن کاملInteresting response of narrow QRS tachycardia to premature atrial complex. What is the mechanism?
Fig.1 shows a premature atrial complex (PAC) introduced during a narrow QRS tachycardia. What is the response and what is the interpretation? The tachycardia is a regular RBBB tachycardia with cycle length of 440 ms and ventriculo-atrial interval of 30 ms. There is near simultaneous activation of atrium and ventricle during tachycardia. Presence of His deflection prior to the QRS makes ventricu...
